Welcome to your medical home! Thank you for choosing our practice and providers to help take care of you and your family’s health. Our practice specializes in Family Medicine. In addition to primary care, we provide DOT physicals for drivers of commercial vehicles.
Family medicine is a medical specialty devoted to comprehensive health care for people of all ages. It is a division of primary care that provides continuing and comprehensive health care for the individual and family across all ages, genders, diseases, and parts of the body. It is based on knowledge of the patient in the context of the family and the community, emphasizing disease prevention and health promotion.
At Boonville Health Center, our goal is to help you maintain your family’s good health. We are committed to providing you with the best treatment regimen that best suits your needs and goals across various chronic disease management diagnosis such as COPD, diabetes, heart disease, asthma, obesity and arthritis.
We strive to improve the health of our patients and community and focus on quality prevention which includes ensuring you and your family’s annual wellness exam (physicals and well child visits) are up to date, you have been educated on preventative health screenings important to you and immunizations to prevent disease and we have shared with you the necessary tools in achieving your health goals.
We understand that when you or a member of your family becomes sick, it can be very stressful. We offer sick visit appointments and same day scheduling to ensure that your immediate needs are addressed in a timely manner. We commit to provide you with appropriate testing, diagnosis, prescriptions, and appropriate follow up care.
Advice During Office Hours
Please call (315) 942-3500 and press the appropriate corresponding number, in reference to the reason for your call. If you have a non urgent health concern, we will record the details of your message/concern and send the message to a member of your Care Team. Every effort is made to respond to you within 24 business hours of your call.
After Hours Care
A physician is available on call, outside of practice business hours, to address your after hour care health concern. Call (315) 942-3500 to be connected with our answering service. A detailed message will be taken and the answering service will contact the on-call provider for you and provide them with the message of your health concern.
At Boonville Health Center, as your Patient Centered Medical Home we work in care teams. What does this mean for you?
Your health care and treatment goals are approached through teamwork. Your Care Team consists of those who work closely within your care – Your Provider, your nurse, your medical record and referral coordinator, your family or care giver, and the specialists that may be involved within your care. Together as a PCMH, our focus is to provide you with better coordinated, quality and comprehensive care.
Billing Information
We participate with most health plan networks. Please click on Insurance Plans to review a detailed list. Please confirm that we accept your insurance, before scheduling your appointment. Your copayment will be due at the time of your appointment. If you have any questions relating to payment or billing, please call (315) 337-1200. We provide equal access to all of our patients, regardless of the source of payment.

Medication Refills
Send us your prescription refill by calling your pharmacy and have them send us the refill request. Please prepare in advance and allow 48 hours for processing requests. If you have questions about your medication, please call us (315) 942-3500.
When You’re in the Hospital
To provide you with the very best care during your hospital stay at Rome Health, your primary care physician has entrusted a hospitalist from St. Joseph's Hospital to be your “in-hospital” doctor.
Hospitalists are physicians who work solely in the hospital and are available throughout your stay to talk with you and your family about your care and treatment.
In case of emergency, your hospitalist is never very far. That’s because he or she works only in the hospital and will see you more than once a day if needed. Hospitalists expedite your care by following up on tests and adjusting your treatment regimen throughout the day based on those test results.
At the time of your admission to the hospital, the hospitalist will review your medical records and may contact your primary care physician to discuss your condition. During the course of your hospitalization, the hospitalist and primary care physician may talk further regarding your treatment, and at discharge, the hospitalist will communicate with your primary care physician to discuss your further treatment needs, help arrange follow-up care, and prescribe the necessary medications.
Your primary care physician will have access to all your records following your stay.

For your convenience, Rome Health offers routine lab testing services on site at Boonville Health Center.
